Patient Information

Appointment

The first step towards a beautiful, healthy smile is to schedule an appointment. New Patient Appointments typically include a cleaning, exam and xrays. Adults - 75 minutes — Children - 45 minutes. 

  • Patients with extensive restorative or other issues may require additional time for diagnostic and treatment planning. Frequently, this requires an additional appointment.
  • Adults who haven’t had a recent cleaning may require additional cleaning appointments.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

   
Dental Insurance

We take most private insurance plans except for capitation plans and DSHS plans (see below).

No Dental Insurance — Our fees are designed to make care as affordable as possible for those without insurance.
Senior and Cash Discounts — 10%

  • No, you can’t combine them.

We are not accepting new DSHS, Applecare, Molinacare, Provider One, and WA Basic Health Plan coverages.
Online Fees — compare our fees with other clinics.

  • The Online Fees are estimates. They are subject to change and insurance contracts.
  • Call Mary at the Front Desk for the latest fees.
